If you’re applying for a new passport from the USA, you can use the DS-11 form because it has a specific section for misplaced and stolen passports. You can then contact your nearest licensed passport agency and passport office to send your documents for attestation and submission.
You can make an online appointment for your passport application. However, if you have to travel within three to four weeks, visit your passport agency in person instead. They’ll review your documents and application, point out any errors and advise how to fix them, and if everything checks out, they’ll process your application on the same day.
